Which US States Accept NCCAOM Certification (Now NCBAHM) and Canadian Training?

Most US jurisdictions use NCCAOM certification or examinations when licensing acupuncturists, but no national certificate gives someone an automatic right to practise across the country.

Forty-seven states and the District of Columbia have acupuncture practice acts. California licenses through its own examination system, while Alabama, Oklahoma and South Dakota do not offer an independent acupuncture licence. Every other licensing jurisdiction uses national board examinations, certification, or both as part of its process. 

NCBAHM became the National Certification Board for Acupuncture and Herbal Medicine in January 2026. Existing certifications remain valid, and many statutes, board forms, and search results will continue to use “NCCAOM” until each state updates its language. The two names refer to the same credentialing organization. 

The distinction that matters is this:

NCBAHM certifies practitioners. State boards license them.

Passing the national examinations, or even holding active NCBAHM certification, does not itself create a licence. The applicant must still satisfy the education, documentation, legal, language and state-specific requirements of the jurisdiction in which they intend to work. NCBAHM itself makes clear that certification neither awards a state licence nor guarantees that one will be issued. 

Acupuncture Licensing Requirements by State

The matrix below is designed as a lookup tool rather than a substitute for legal advice. The NCBAHM category describes the main entry route, not every condition attached to the licence.

  • Full certification means the state generally requires an NCBAHM Diplomate credential or comparable current certification.
  • Examination route means the state relies primarily on specified NCBAHM examination results and may not require the practitioner to maintain full certification.
  • Route 2 refers to the NCBAHM international education pathway used by graduates of Canadian and other non-US schools.
  • Additional review means the state may separately assess education, clinical hours, language proficiency, jurisprudence or other documents.

One accuracy issue deserves to be visible. Current NCBAHM pages are not fully synchronized: the organisation’s State Relations page reports 22 certification states and 26 examination states, while its Board Examination Process page reports 25 certification states. The individual board’s current rule should therefore take priority over any national count.

Requirements can change between application cycles. Before paying for a credential evaluation, examination or state application, obtain written confirmation from the relevant board.

Full Certification and Examination Routes Are Not the Same

A state requiring full NCBAHM certification is asking for more than a passing score report.

To become a Diplomate, an applicant must meet the education pathway, complete Clean Needle Technique requirements, pass the required board examinations and satisfy the organisation’s documentation standards. Maintaining an active credential can then involve recertification and continuing professional development.

An examination-route state may instead ask NCBAHM to send proof that the applicant passed specific modules. The state can use those results as evidence of entry-level competence without requiring the person to maintain active Diplomate status indefinitely.

That distinction affects:

  • application sequencing
  • examination modules
  • recurring certification costs
  • continuing education
  • relocation planning
  • whether an expired national credential creates a state licensing issue

The certification may be portable. The licence is not.

California Operates Outside the National Licensing Route

California is not simply another examination-route state. It runs a separate licensing system through the California Acupuncture Licensing Examination, or CALE.

The California Acupuncture Board does not accept NCBAHM examination passage as a substitute for the CALE and does not grant acupuncture licences through ordinary reciprocity. Someone already licensed in another state must still qualify under California’s education rules and pass the state examination before practising there. 

For a Canadian-trained applicant, California requires a course-by-course evaluation through a Board-approved credential evaluation service. The Board then compares the applicant’s education and clinical experience with California’s curriculum standards. Missing coursework may have to be completed before examination eligibility is granted. 

The CALE is offered in English, Chinese, and Korean. However, language availability does not reduce the underlying education and clinical-training requirements.

California therefore needs to be treated as an education-planning decision, not merely an exam choice at the end of a Canadian program.

A Canadian Diploma Goes Through Two Separate Reviews

A Canadian-trained practitioner does not convert a Canadian acupuncture licence into a US one.

The normal path contains at least two evaluations.

1. NCBAHM Determines Examination Eligibility

Canadian education is treated as international education because it was completed outside the United States.

Under NCBAHM’s international Route 2, an applicant must hold an acupuncture, TCM or herbal medicine degree or diploma from an institution that meets the organisation’s oversight standards.

NCBAHM requires a course-by-course evaluation from International Consultants of Delaware, or ICD. The process examines the authenticity and distribution of the applicant’s academic and clinical hours. An original-language transcript and an English translation must be supplied where applicable. NCBAHM no longer accepts World Education Services evaluations for this route. It warns that foreign education review can take approximately six months. 

The result is not an American licence. It establishes whether the applicant is eligible to enter the national examination and certification process.

2. The State Makes Its Own Licensing Decision

After the national stage, the applicant submits a separate state file.

Depending on the jurisdiction, that file may include:

  • official NCBAHM exam or certification verification
  • original school transcripts
  • a second credential evaluation
  • documented clinical hours
  • Clean Needle Technique certification
  • a criminal background check
  • a jurisprudence examination
  • English-language evidence
  • licence verifications from Canada or other jurisdictions
  • Social Security, immigration or lawful-presence documentation

Several states make these additional requirements explicit. The District of Columbia application, for example, requires TOEFL evidence where the applicant’s education was conducted in another language. Delaware’s licensing route can require TOEFL where English is not the applicant’s first language, while Florida evaluates foreign-equivalent education and may require English evidence and state-specific coursework.

A Canadian program can provide the academic foundation for this process. It cannot bind NCBAHM or a state board to accept an individual application.

The US Has Credential Portability, Not One National Licence

“Reciprocity” is often used too loosely in acupuncture.

A practitioner moving between states usually carries four things:

  • education
  • NCBAHM examination results
  • national certification
  • professional history

The receiving state then decides whether those credentials satisfy its practice act.

Some jurisdictions offer endorsement or reciprocity applications for practitioners already licensed elsewhere. That can reduce duplication, but it is not automatic recognition. Delaware, for example, requires the other jurisdiction’s standards to be substantially equivalent. California offers no ordinary reciprocity at all.

A Canadian provincial registration can strengthen an applicant’s file by demonstrating good standing and lawful professional practice. It does not occupy the same legal position as a licence issued by another US state, and it does not remove the NCBAHM or education-review stage.

There is also a separate issue that licensing articles often ignore: professional eligibility is not immigration status. Passing examinations and receiving a state licence do not independently provide a visa, employment authorisation or permission to work in the United States.

No Practice Act Does Not Mean Unrestricted Practice

Alabama, Oklahoma and South Dakota do not issue an independent acupuncture licence under a dedicated practice act.

That does not create an open market in which any acupuncture graduate can establish an unrestricted practice. Needling may be prohibited, limited or performed only under the legal authority of another licensed profession, such as medicine or chiropractic.

For a Canadian-trained acupuncturist seeking independent practice, these states provide no conventional route comparable with Texas, Florida or New York. Holding NCBAHM certification does not create a state licence where the legislature has not established one.

Choosing Canadian Training With US Practice in Mind

Students considering both Canada and the United States should choose education around the strictest realistic destination, not the easiest place to begin working.

Before enrolling, ask:

  • Does the program’s content and clinical training support NCBAHM Route 2 review?
  • Can the school provide detailed course descriptions and clinical-hour records?
  • Will official transcripts be available directly to credential evaluators and boards?
  • Does the program prepare students for the biomedical and practice-management content tested nationally?
  • Could the curriculum meet a demanding state’s education review?
  • Is California a serious goal, or only a vague possibility?
  • Will the intended state require full certification, selected exams or additional state testing?

How Many US States Require NCCAOM or NCBAHM Certification?

Current NCBAHM materials are inconsistent on the exact split. One page reports 22 full-certification states, while another reports 25. What remains clear is that 46 states plus the District of Columbia use NCBAHM certification or examinations in some form, while California uses the CALE. Applicants should rely on the individual board rule rather than a national count. 

Which States Do Not License Acupuncturists Independently?

Alabama, Oklahoma and South Dakota do not have independent acupuncture practice acts. NCBAHM certification alone does not create permission to practise independently in those states.

Does California Accept NCCAOM Certification?

No. California requires applicants to qualify for and pass the CALE. Passing NCBAHM examinations or holding another state’s licence does not replace California’s process. 

Can a Canadian-Trained Acupuncturist Get Licensed in the US?

Yes, provided the applicant’s education satisfies the NCBAHM international pathway and the chosen state’s requirements. The process may include credential evaluation, national examinations, certification, state testing, language evidence and other application documents.

Is There Acupuncture Licence Reciprocity Between US States?

Some states offer endorsement or reciprocity routes, but there is no universal transfer system. The receiving state determines whether the applicant’s existing licence and qualifications are equivalent to its own standards.

Do State Laws Still Say NCCAOM?

Many do. The organisation changed its name to NCBAHM in January 2026, but updating references across dozens of statutes and regulations will take time. The old and new names currently identify the same organisation, and existing credentials remain valid. 

Plan for the State, Not Only the Certificate

NCBAHM is the closest the profession has to a national competency framework, but it does not erase state borders.

The state determines which credential is required, which examination modules count, whether foreign education is equivalent and what additional conditions apply. California stands outside the national examination route, three states offer no independent licence, and internationally trained applicants face an education review before state licensing even begins.

For Canadian-trained practitioners, the strategic order is straightforward:

Choose the state. Map its education and examination requirements. Complete the international credential review. Then build the licensing application around that jurisdiction.
